Pengantar Training :
One of the principle changes with respect with the OHSAS 18001 2007 is the term “accident” is now included in he term “incident”. Based on the above reason it is important that all incidents, accidents, injuries, and illnesses be investigated in order to determine if steps can be taken to prevent or reduce the likelihood of a similar event in the future. These investigations will be conducted using a graded approach that takes into account the actual and potential severity and consequences of the incident.

In either case, prompt and complete investigation of incidents and accidents are keys to identifying the causes and establishing effective corrective actions to prevent recurrence.

The purpose of this workshop is leading you to leave this workshop knowing how to conduct an incident investigation and properly complete an accident investigation report with confidence using our systematic approach.

Profession Member Indonesian Association of HSE Specialist in Chemical Safety. Profession Member Indonesian Association of Occupational Hygiene and Occupational Safety.

RELEVANT EXPERIENCE: Over the past 29 years Bambang Pamudji has provided HSE management program development and implementation as well as risk assessment leadership ,HSE Management System Internal Auditor on PT. Pupuk Sriwidjaja Urea Fertilizer Company Palembang. He has led “step change” HSE improvement in all assignments. On The Last past 4 years Bambang Pamudji has work on his current project as Advisory service in HSE Management System,HSE Training program Development and trainer, HSE Auditor, HSE Coordinator in a variety of areas ranging from oil/gas production, pipeline operations, and chemical plants.
